Services provided include a Housing Resource Center which is a one-stop-shop with answers to housing questions. Services in the Housing Resource Center include: Help with assessing current housing situation, identifying needs and targeting resources to reach housing goals; Information about housing opportunities; Access to roommate database and Section 8 Alert list; Information about homeowner, tenant and landlord rights and responsibilities; Future Housing Planning for individuals/families with disabilities; Access to Housing Resource Manual; and Independent Living Club. PATH provides assistance in locating appropriate housing for individuals and families who qualify. PATH can also provide some financial assistance for placement costs and can help to secure furniture and other necessary household items. PATH provides referrals and links consumers to other appropriate agencies such as the Department of Human Services, Social Security, Michigan Works, SMART Transportation, local food banks, and any other programs that might be needed to ensure a successful transition. ***QUALIFICATIONS FOR THE PATH PROGRAM: Individuals MUST BE HOMELESS. Individuals must be able to provide documentation verifying his or her homelessness. Individuals MUST HAVE A CHRONIC AND PERSISTENT MENTAL ILLNESS and must be willing to engage in treatment for that illness. Individuals must possess appropriate skills and MUST BE CLINICALLY ABLE TO LIVE INDEPENDENTLY. Individuals MUST HAVE A SECURED INCOME or must be able to provide documentation that he or she will have income within 30 days of acceptance. Individuals MUST BE COOPERATIVE WITH TREATMENT RECOMMENDATIONS made by their current treatment team, as well as the PATH Housing Specialist*** If you have a consumer who may qualify for the PATH program, or if you would like more information, please contact: Connie Lasher, MCCMH Housing Specialist (586-469-6152), or Diane at Liberties (586-777-8094). Along with PATH, a Rapid Rehousing Grant that provides similar services as PATH is offered, as well as the SOAR Program which helps PATH eligible participants who have no income and are unable to work due to their psychiatric issues. (VCCF) Veterans Constructing Communities Foundation is a Michigan registered 501(c)(3) nonprofit organization created to serve veterans of all war eras, their families and surviving spouses’ housing needs. VCCF introduced the first housing project for the medically fragile and cognitive impaired. As we grow this model, we will also introduce various housing options from single family homes to aging in place in their own living environment. VCCF works closely with a multitude of human resource service providers, nonprofits organizations and governmental agencies to help meet the needs of the veterans. These collaborative efforts help sustain their success, no matter what stage of housing is needed.
